La historia se sitúa 10 años después del Bionic Commando original, tomando control del mismo protagonista, Nathan Spencer (Con la voz de Mike Patton- Faith No More), ahora convertido en presidiario por error, claro está. Privado de su arma biónica, Spencer vuelve a la libertad tras sufrir Ascension City un brutal ataque nuclear que diezma su población y devasta su morfología; la ciudad que encontramos es un reflejo del estereotipo de holocausto post-nuclear: edificios medio demolidos, mostrando sus estructuras, trozos de carreteras colgando a varias alturas y puentes derribados bajo un cielo anaranjado.
