Dos chicas americanas que viajan a través de Europa tendrán una avería en el coche en mitad de unos bosques en Alemania buscando ayuda llegarán a una casa aislada y al día siguiente despertarán atrapadas en un tétrico sótano de un hospital junto a un hombre japonés.
Poco después descubrirán que un alemán de avanzada edad que anteriormente era un cirujano especializado en separar a gemelos siameses.
